The procedure

Surgery that lifts sagging breasts and, as part of the same plan, adds volume with implants: it removes extra skin, reshapes the breast tissue, and raises the areola and nipple.

At your in-person consultation, Dr. Fernando Ruiz decides with you on your scar pattern, the size and placement of the implant, and whether everything is done in one surgery.

Is it for you

What you're looking for

  • Breasts that sag and have also lost volume
  • An 'empty' upper breast after pregnancy, breastfeeding, or weight loss
  • Areolas and nipples that sit low or point downward

Your surgeon makes the final call at your consultation.

Recovery

Step by step

You leave wearing a post-surgical bra. You need someone with you.

Take it easy, care for your incisions, and come in for your first follow-up visit.

A gradual return to your routine, with no strenuous activity.

The implants settle, and around month 6 you see the final result. Your scars keep fading through the first year.

These are guideline times. Your surgeon gives you the exact ones.

Before you decide

All about a breast lift with implants

Breast lift with implants or augmentation mastopexy: the same surgery

You may also see it called a breast lift with augmentation, or breast lift and augmentation. In Colombia it's known as mastopexia con implantes or pexia con implantes. It raises a sagging breast and, as part of the same plan, adds volume. It's one form of breast lift, which can also be done with your own tissue alone. According to the American Society of Plastic Surgeons (ASPS), a lift on its own doesn't significantly change breast size or fill out the upper breast. If you want a fuller look, the ASPS suggests adding an augmentation.

What changes compared with a breast lift alone

The lift itself is the same: extra skin is removed, the tissue is reshaped, and the areola and nipple are moved higher. What's added is the implant. The FDA has approved two types, saline and silicone gel, and according to the ASPS, the implant can go under or over the pectoral muscle. The ASPS explains that the lift can often be done in the same surgery as the augmentation, though sometimes it requires a separate operation. Surgery takes 3 to 5 hours, under general anesthesia. Your surgeon decides how it's done in your case.

Who it isn't for, or not yet

Based on what the ASPS says about candidates for a lift and for an augmentation, you may be a candidate if you're healthy, your weight is stable, you don't smoke, your breasts are fully developed, and your expectations are realistic. It's better to wait, or to think it over carefully, if:

  • You're pregnant or breastfeeding.
  • You're planning more pregnancies: according to the ASPS, they can reduce or undo the lift.
  • Your weight is still changing.
  • You're doing it for someone else: it should be your decision.

Your surgeon makes the final call at your in-person consultation.

Risks you should know about

Every surgery has risks. These are some the ASPS lists for a breast lift and for augmentation:

  • Anesthesia risks, bleeding, hematoma, infection, and fluid buildup.
  • Asymmetry, poor scarring, and changes in nipple or breast sensation.
  • Fat necrosis, and partial or total loss of the nipple and areola.
  • Capsular contracture, and implant rupture or malposition.
  • A lymphoma associated with breast implants (BIA-ALCL), or other very rare cancers in the capsule.
  • Deep vein thrombosis, heart and lung complications, and possible revision surgery.

We go over each one with you, and you sign an informed consent form.

How to prepare, and what recovery is like

Before surgery, you get your pre-op tests, stop smoking, and review everything you take: according to the ASPS, aspirin, anti-inflammatory drugs, and herbal supplements can increase bleeding. We'll tell you what to stop taking and what to bring. You leave wearing a post-surgical bra, and you need an adult to stay with you the first night. The first week is for taking it easy and caring for your incisions. Between weeks 2 and 4 you ease back in, with no strenuous activity. The implants settle between months 3 and 6. If they're silicone, the FDA recommends periodic screening.

When another option makes more sense

You don't always need both.

Your surgeon decides this at your in-person consultation.

Coming from abroad

Plan your trip.

If you are flying in, plan to stay in Medellín for about 1 to 2 weeks for this surgery, and fly home only when your surgeon clears you. Travel with an adult companion for the first days.

FAQ

Common questions

Is it all done in one surgery?

Often, yes. According to the ASPS, the lift can be done in the same surgery as the augmentation, although sometimes it requires a separate operation. Your surgeon decides this at your in-person consultation.

How do I know if I also need a lift?

An implant adds volume, but it doesn't lift: the ASPS makes clear that augmentation doesn't correct severely sagging breasts. Among the signs it links to a lift are nipples that sit below the breast crease when you're not wearing a bra, and areolas and nipples that point downward. Your surgeon confirms this during your exam.

How long is the recovery from a breast lift with implants?

You'll need about 2 to 3 weeks off work. The first week is for taking it easy and caring for your incisions, and between the second and fourth week you gradually return to your routine, with no strenuous activity. The implants settle between months 3 and 6, and you see the final result around month 6. If it's combined with another surgery, the timing changes: your surgeon decides this at your in-person consultation.

Is there a scarless breast lift with implants?

No. Lifting the breast means removing extra skin, and that leaves a scar. According to the ASPS, the scar can go around the areola only, around the areola and straight down to the crease beneath the breast, or along that crease as well. That last pattern is known as an anchor scar. Which one you need depends on how much lift is required. The scars are permanent. In most cases they fade over time, and they keep fading for up to a year.

How long do the results of a breast lift with implants last?

According to the ASPS, the results of a lift are long-lasting, but your breasts keep changing with age and gravity, and a pregnancy or significant weight loss can affect them. Implants aren't lifetime devices either: the FDA explains that there's no way to predict how long an implant will last, and that the longer you have implants, the more likely you are to need them removed or replaced.

Can I get small implants with a breast lift?

Yes. The implant doesn't have to be big: implants come in a range of sizes, and yours is chosen based on the width of your chest, your skin, and what you're looking for. Sometimes the goal is simply to fill out the upper breast. Your surgeon decides this at your in-person consultation.
