The procedure

It combines a tummy tuck, which removes extra skin and fat from the abdomen and repairs diastasis recti when needed, with liposuction to define the waist and the transfer of that fat, once purified, to your buttocks.

It's a bigger surgery than any of the three on its own. Your surgeon decides at your in-person consultation whether everything is done together or in separate stages, and your test results confirm it.

Is it for you

What you're looking for

  • Extra skin on your abdomen, or diastasis recti
  • Fat at your waist or back that can be moved to your buttocks
  • A stable weight, good health and no plans for more pregnancies

Your surgeon makes the final call at your consultation.

Recovery

Step by step

You walk slightly bent forward, wear a compression garment, keep activity light and have a companion.

No direct pressure on your buttocks, as directed. Compression garment and massage.

Gradual return to exercise, once you're cleared.

The volume in your buttocks is stable, and the scar on your abdomen matures.

These are guideline times. Your surgeon gives you the exact ones.

Before you decide

All about lipoabdominoplasty with BBL

Tummy tuck and BBL, or lipoabdominoplasty with BBL: what it is

Tummy tuck and BBL, tummy tuck with lipo and BBL, lipoabdominoplasty with BBL or, in Spanish, lipoabdominoplastia con transferencia glútea: they're all names for the same combination of three surgeries. A tummy tuck removes the extra skin and fat on the abdomen. Liposuction defines the waist and back. And a Brazilian butt lift (BBL) moves that fat, once purified, to the buttocks. It isn't a fixed package: what gets combined depends on your body and your test results.

What changes compared with each surgery on its own

Each part has its own goal. On the abdomen, the extra skin is removed, the muscles are repaired if there's diastasis, and the scar sits low, covered by underwear. In the buttocks, the fat is placed above the muscle, never inside it. The difference is how big the surgery is: on its own, a tummy tuck takes 3 to 4 hours and a BBL 3 to 5 hours; together, it takes longer. Recovery also means caring for two areas at once. Plan ahead for time off work and for who will take care of you in the first days.

Who it isn't for, or not yet

According to the American Society of Plastic Surgeons (ASPS), a good tummy tuck candidate is in good health, at a stable weight, has realistic expectations and doesn't smoke. For fat transfer to the buttocks, the ASPS adds being close to your ideal weight. It's better to wait if:

  • You plan to lose a lot of weight: that comes first, with a medical weight-loss plan.
  • You want more children.
  • You have little fat elsewhere: there may not be enough to transfer.
  • You smoke and won't quit beforehand.
  • You expect to erase cellulite or all your stretch marks.
Risks: why combining calls for extra care

Here the risks of three surgeries add up. For a tummy tuck and liposuction, the ASPS lists, among others, infection, poor wound healing, fluid buildup, deep vein thrombosis and heart or lung complications. In the buttocks, the most serious risk is fat embolism, which can happen if fat is accidentally injected into the muscle. Reviewing deaths after cosmetic surgery, the CDC identified obesity and having several procedures in the same operation as risk factors for embolism. That's why we don't operate until your pre-op tests are in order, and you sign an informed consent form.

How to prepare, and aftercare

Beforehand, you get pre-op tests, stop smoking and review your medications: according to the ASPS, aspirin, anti-inflammatory drugs and herbal supplements can increase bleeding. We tell you what to stop taking. An adult stays with you from the first night. The first week, you walk slightly bent forward, in a compression garment. Through week 3, you avoid direct pressure on your buttocks, as your surgeon directs, and for sitting, the ASPS recommends a cushion under the back of your thighs. If you have shortness of breath, chest pain or an irregular heartbeat, get medical care right away.

When another option makes more sense

You don't always need all three. Depending on what bothers you, other options include:

Coming from abroad

Plan your trip.

If you are flying in, plan to stay in Medellín for about 2 to 3 weeks for this surgery, and fly home only when your surgeon clears you. Travel with an adult companion for the first days.

FAQ

Common questions

Can I have children afterward?

Yes, but ideally you have surgery once you're no longer planning more pregnancies, so you keep your result.

How much of the volume in my buttocks stays?

Part of the fat is reabsorbed. What integrates in the first few months is permanent.

Lipoabdominoplasty vs. tummy tuck: what's the difference?

A tummy tuck removes extra skin and fat from the abdomen and, if needed, repairs diastasis recti. When liposuction is added to define the waist, it's called a lipoabdominoplasty. If that fat is also moved to your buttocks, it's a lipoabdominoplasty with BBL, often simply called a tummy tuck and BBL. Your surgeon tells you which one you need after examining you.

Can you get a tummy tuck and BBL at the same time?

You can, but not in every case. Together they make a bigger surgery, and the CDC points to obesity and having several procedures in the same operation as risk factors for embolism. Your surgeon decides at your in-person consultation, with your test results, whether everything is done at once or in separate stages.

How do you sleep after a tummy tuck and BBL?

Two kinds of care overlap here. For your buttocks, the ASPS recommends sleeping face down or on your side. When the abdomen is tightened, the ASPS says you may be asked to avoid standing fully upright and to sleep with pillows elevating your knees. Before surgery, your surgeon tells you how to position yourself for both. Don't work it out on your own.

How long does a tummy tuck and BBL take?

It depends on what's combined. On their own, a tummy tuck takes 3 to 4 hours and a BBL 3 to 5 hours, both under general anesthesia; together, the surgery takes longer. Time off work for each one alone is 2 to 3 weeks, and combined it can be more. Your surgeon gives you the timing for your case at your in-person consultation.
